Sample 1 & 2
O.D. = Right Eye
O.S. = Left Eye
SPH= Sphere power. The overall power of the lenses
CYL=Cylinder power It is the amount of astigmatism correction. Some prescriptions are written
in (-) cylinder and some are written in (+) cylinder. There are two different ways of writing
this in a prescription. Some doctors write their prescriptions in plus (+) cylinder and some
in minus (-) cylinder. They are not the same. Please take special precaution when
entering your prescription when you order your eyeglasses.
AXIS=axis X80 means Axis 080
PRISM=
ADD=The amount of power that gets added to the distance prescription (this creates your reading
only prescription) or for the lower portion of your bifocals/progressives
Sample 3
This type of prescription formats the left (OS) and right (OD) eyes into two categories:
Distance (Dist.) usually abbreviated D.V.
Near (Reading) usually abbreviated N.V.
Additional Information Concerning Your Prescription:
-D.S or DS = diopters sphere (means you have sphere power only)
-NVO or NV = near vision only or near vision; your doctor is recommending reading only glasses
-Your prescription may be written on a blank Rx pad. The top number is the prescription for
the right eye (OD) and the bottom number is the prescription for the left eye (OS).
-If there are no numbers for CYL, AXIS or ADD in your prescription, leave these fields blank
when ordering. When only one number appears for each eye it is for sphere power only.
-There are times when your prescription will not include decimal points. This means that +125=+1.25
and -85=-0.85
